Medical Records and Patient Confidentiality
Orthopedics International values your business and respects your rights to privacy. Each employee and service provider has signed a confidentiality agreement to protect our patients.
Private information you will need to provide
You will be asked to give private information including your name, address, date of birth and Social Security number. We will verify insurance information at each visit. Many insurance companies are moving away from using your Social Security number on your insurance cards to help prevent identity theft. However, we will still request this information since it is required for billing purposes.

Orthopedics International follows all current HIPAA regulations to assure that your private medical information remains confidential.

Medical records retention
We will retain your medical records for five years.
Requesting your records
You or your representative may request your medical records. In order to release your records, we need written approval from you. This requires you to fill out a specific form with your signature and current date. Please note that there may be fees associated with these requests.
