• Premera (includes Foundations Plus 1 but not Premera Foundation plan)

• First Choice Health Network
• Uniform Medical Plan
• Self Insured Workers Compensation *
• Labor & Industry of Washington State *
• Medicare *
• Med Advantage Plans (some)
• Auto Accidents*
• Many out-of-state plans with proper patient authorizations 

* Limited availability with many physicians

Note: If your insurance requires a referral from another physician, you are responsible for making sure that this has been obtained by the time of your visit.

Insurance plans that are NOT accepted

• Cigna Insurance plans (Orthopedics International will see patients with Cigna Insurance on an “out of network” basis; patients are given a discount on services rendered if they pay in full at the time of service.)
•  PHCS Private Healthcare Systems
• DSHS of Washington State
• Community Healthplan of WA
• Molina 
• Washington State Crime Victims
• Third-party payors including auto and homeowners insurance plans
