What to Bring to Your Appointment
- Insurance card and photo ID: These must be current.
- Test results: This includes previous X-rays, CT scans, MRI scans, or other test results related to your medical problem.
- Medical records: These should relate to your current medical problem.
- Referral authorization: If necessary, this should be from your primary care provider (i.e., HMO referrals). If you are unsure if this is required by your insurance, please call them prior to your appointment.
- Payments: For co-pay and self-pay patients the payments are due at the time of service. We accept payment by credit card (Visa and MasterCard), personal check or cash.
- Worker’s compensation claim: If this is a worker’s compensation claim, we will need your:
- claim number
- date of injury
- Self-insured claim: If this is a self-insured claim, in order to ensure proper billing for your claim, you must provide us with the:
- name of the self-insured company
- billing address of the self-insured company
- Motor vehicle accident: If you were involved in a motor vehicle accident and made a claim with your insurance company, we will need your:
- claim number
- date of injury
- claim adjustor name
- claim adjuster phone number
