What to Bring to Your Appointment

 

  1. Insurance card and photo ID: These must be current.
  2. Test results: This includes previous X-rays, CT scans, MRI scans, or other test results related to your medical problem.
  3. Medical records: These should relate to your current medical problem. 
  4. Referral authorization: If necessary, this should be from your primary care provider (i.e., HMO referrals). If you are unsure if this is required by your insurance, please call them prior to your appointment.
  5. Payments: For co-pay and self-pay patients the payments are due at the time of service. We accept payment by credit card (Visa and MasterCard), personal check or cash.
  6. Worker’s compensation claim: If this is a worker’s compensation claim, we will need your:
    • claim number
    • date of injury
  7. Self-insured claim: If this is a self-insured claim, in order to ensure proper billing for your claim, you must provide us with the:
    • name of the self-insured company 
    • billing address of the self-insured company
  8. Motor vehicle accident: If you were involved in a motor vehicle accident and made a claim with your insurance company, we will need your: 
    • claim number
    • date of injury 
    • claim adjustor name
    • claim adjuster phone number
